Summary:The pharmacotherapy and device therapy for heart failure with reduced ejection fraction (HFrEF) have advanced considerably. However, there are instances where even with optimal therapy following the guidelines’ recommendations, some patients may require heart transplantation (HTX) or left ventricular assist device implantation due to the progressive nature of the disease. Here, we share the case of a young man who showed signs of progressive heart failure ten months after diagnosis, leading to an urgent investigation and eventual successful HTX in the twelfth month post-diagnosis.
