description Serous cystadenoma of the ovary is a benign epithelial neoplasm in most cases. Its size varies and ranges from three to ten centimeters in length. It is relatively rare, slow-growing and of unknown cause. It usually occurs in adult women of all ages, with an average age of 40 to 60 years. A case of a 35-year-old patient with a history of apparent health, who went to the Surgery Emergency Room of the Dr. Gustavo Aldereguía Lima Hospital in Cienfuegos because for several months she had been experiencing progressive abdominal volume increase, in addition to persistent pain in the lower abdomen that radiated to the legs, accompanied by difficulty defecating and urinating on occasions is presented. On physical examination, a hard, mobile and somewhat painful mass was found, occupying the lower hemiabdomen; with dullness to percussion. Vaginal examination revealed a painful left adnexal mass when the neck was moved. Abdominal ultrasound revealed a large heterogeneous image measuring 35x24 centimeters, which compressed and displaced neighboring organs; and a small amount of free fluid at the bottom of the bag, which revealed a left ovarian cyst. The tumor was excised. Six months after surgery, the evolution was still favorable. The diagnosis of this entity is complex; it is made during laparotomy, due to the absence of specific symptoms and signs that allow malignancy to be ruled out.
