    “…In this case, it is clear that multiple myeloma treatment optimization cannot be achieved by considering multiple myeloma treatment only as applying the appropriate drug combination.In order to ensure the optimization of IMID-based therapies, it is necessary to take a broad perspective.This perspective should include the correct selection of the patient to be treated with IMID, the development and implementation of IMID forms that will increase patient compliance, the use of techniques to predict the development of resistance, side effect management during treatment, preventing the increase in disease burden with appropriate maneuvers by following the disease burden after treatment, and drug-free follow-up.It is recommended to avoid thalidomide in the presence of peripheral neuropathy, to prefer other agents instead of lenalidomide in the presence of renal function loss, and to avoid pomalidomide in patients with COPD.Nex-20, a once-monthly subcutaneous formulation of lenalidomide, is expected to be an important breakthrough in improving patient compliance.Prediction of which patients will develop resistance will be very useful in order to have the best treatment outcomes.Calculation of miRNA risk scores, measurement of lenalidomide metabolite levels in urine, measurement of cereblon levels in blood, or immunohistochemical detection of cereblon levels are some of the methods currently being tried to predict disease resistance.Patients who develop anemia should be treated with erythropoietin, skin reactions should be treated with steroids and antihistamines, diarrhea should be treated with loperamide and colestyramine, and in general, the drug should be discontinued and the same dose or a lower dose should be insisted upon.An important part of treatment is maintenance therapy with lenalidomide. …”

    “…Objective To investigate the related factors and prevention of acute pancreatitis(AP) complicated by uremia.Methods We retrospectively reviewed the medical records of all patients with AP complicated by uremia who were hospitalized between January 2009 and June 2014.According to whether or not suffering AP,they were divided into AP group and control group.For two groups,the baseline characteristics and laboratory measurements were collected and analyzed.Results During the study period,1 665 eligible patients were enrolled,30 patients had AP,the overall prevalence rate was 1.80%,and incidence of AP was 3.28 per 1 000 person-years.The prevalence rate was0.92%,and incidence of AP was 1.67 per 1 000 person-years in the hemodialysis group.The prevalence rate was 1.20%,and incidence of AP was 2.19 per 1 000 person-years in the peritoneal dialysis group.The prevalence rate was 3.17%,and incidence of AP was 5.77 per 1 000 person-years in the non-dialysis group.The AP group had marked differences in leukocyte count,renal function,carbon dioxide-combining power,calcemia,phosphorus,intact parathyroid hormone(iPTH),liver function,high sensitivity C-reactive protein(hs-CRP),procalcitonin(PCT) and glomerular filtration rate(GFR) as compared with the control group(P<0.05).Multivariable logistic regression analysis showed that calcemia and GFR were related with AP in uremia patients.Conclusions The hypocalcemia and decline of residual renal function are the related factors for the occurrence of AP in patients with uremia.Clinically calcium and phosphorus imbalance should be prevented,and rGFR should be protected actively to prevent AP.…”
