Effect of self-management support on the progression of primary chronic glomerulonephritis by QIN Xin-dong, WU Yi-fan, LIANG Jue-yao, FU Li-zhe, LIU Xi, LIU Xu-sheng

    Published 2015-01-01
“…Objective Primary chronic glomerulonephritis(CGN) is a continuous progressive chronic disease which is closely related with dietary habits and lifestyles. This study was designed to explore the effect of self-management support(SMS) on the progression of CGN.Methods The retrospective cohort study was conducted on those CGN patients with proteinuria and eGFR ≥30 ml·min-1·(1.73 m2)-1 who regularly consulted in Department of Nephrology.Guangdong Provincial Hospital of Chinese Medicine from January 1,2010 to December 30,2014.Information of demographic data, disease characteristics, laboratory indexes, treatments and SMS was collected.Endpoint events included an eGFR decrease of up to 30%or end-stage renal disease(ESRD) demanding renal replacement therapy. All above factors were statistically dealt with by COX regression model to analyze independent influence factors of incident of endpoint events in CGN patients. Results This study included216 CGN patients. The median follow-up time was 119 weeks(65.75,175.50).Endpoint events occurred in a total of 20 patients. The results showed that SMS[OR= 0.307,95%CI(0.110,0.860)]was an independent protective factor to postpone the progression of CGN.Meanwhile, systolic pressure[OR= 1.024,95%CI(0.999,1.049)]might be a risk factor for CGN patients to endpoint event Conclusions This study showed that SMS may have a positive effect on delaying progression of GCN.We suggest that SMS should become an important part of CGN regular treatment.…”
