Specifically, the study set out to establish the relationship between decentralized planning and health service delivery in Kabale District Local Government; analyse the effect of revenue control on health service delivery in Kabale District Local Government; and, establish how revenue allocation influences health service delivery in Kabale District Local Government. A cross-sectional research design was adopted to collect both qualitative and quantitative data from 137 respondents. Data collection was done using questionnaires, interview guide and documentary checklist; and analysis was done using Statistical Package for Social Sciences (SPSS Version 20). Results indicate a strong positive correlation (r= 0.958**) between decentralized planning and health service delivery. In addition, there was a significant positive relationship between revenue control and health service delivery (r= 0.957**), and a significant positive relationship between revenue allocation and health service delivery in Kabale District Local Government (r = 0.953**). Generally, findings show that there was a significant relationship between decentralized planning and health service delivery in Kabale District Local government. Therefore, the study concluded that revenue enhancement plan/strategies, increased revenue mobilization/collection, participatory planning and budgeting improve on the quality of health service delivery. The results indicated a significant positive relationship between revenue control and health service delivery in Kabale District Local Government. The study concluded that allocating adequate funds to Kabale District Local Government, better allocation and proper use of financial resources in the delivery of quality services and monitoring revenue allocation in health improves on health service delivery. The study recommended that there is a need to strengthen decentralized planning for health services by allowing the input of local policy makers and creation of funding for that input. There is need for Kabale District Local Government to strengthen and increase on revenue management strategies in order to mobilize adequate revenues to finance the delivery of health services.
