description A 73-year-old man was admitted to our hospital with a blunt thoracic aortic injury owing to a traffic accident. We diagnosed traumatic aortic dissection with visceral malperfusion requiring emergency thoracic endovascular aortic repair (TEVAR). The primary entry tear was at the bifurcation of the left subclavian artery in the aortic arch. A chimney TEVAR preserved blood flow in the left common carotid artery and ensured an adequate proximal landing zone. The patient recovered and was discharged on postoperative day 16. Traumatic aortic dissection with visceral malperfusion is rare, but chimney TEVAR was useful in this case.
