Summary:Objective To explore the application of two⁃dimensional speckle tracking echocardiography(2D⁃STE)inpatients with immunoglobulin light⁃chain amyloidosis(AL⁃CA)and its value in predicting subclinical cardiac involvement.Methods Eighteen AL⁃CA patients admitted in the First Affiliated Hospital of Soochow University between January 2020 and June 2023 were selected retrospectively as the study group,and 18 healthy patients with physical examination in the same period were selected as the control group. The 2D ⁃ STE technique was used to obtain the peak systolic longitudinal strain of 17 left ventricular(LV)segments and the left ventricular global longitudinal strain(LVGLS).Conventional echocardiography was employed to measure LV posterior wall thickness(LVPWT),LV end ⁃ systolic volume(LVESV),LV end⁃ diastolic volume(LVEDV),interventricular septal thickness(IVST),left atrial inner diameter(LAID),and LV ejection fraction(LVEF). Differences in these parameters between the two groups were compared. Receiver operating characteristic(ROC)curves were plotted to analyze the predictive value of 2D ⁃ STE parameters for subclinical cardiac involvement in AL⁃CA patients. Results Compared with the control group,the peaksystolic longitudinal strain of 17 segments of LV in study group were significantly reduced(P<0.01),the LVGLS in the study group was significantly reduced compared with the control group[(-13.70 ± 3.30)% vs(-23.75 ± 2.62)%,P <0.01]. ROC curve analysis showed that the area under the curve of LVGLS for prediction of subclinical cardiac involvement was 0.99,the cutoff point for LVGLS was -20.25%,with a sensitivity of 100% and a specificity of 94%.Conclusion In patients with AL⁃CA but without clinical cardiac involvement,2D⁃STE has great value in evaluating the subclinical cardiac involvement,and LVGLS is a valuable and sensitivity parameter.
