description Loculation of air and fluid within a pulmonary fissure is uncommon and may occur due to pleural adhesions following a pleural disease or pleurodesis. A rare case of decompensated congestive heart failure (CHF) with localisation of air and fluid within the oblique fissure (interlobar hydropneumothorax) of the right lung is reported. Clinical features with separate bilateral pleural effusion and high serum N-terminal pro-brain natriuretic peptide suggested the aetiology of interlobar hydropneumothorax to be CHF. Computed tomography of the chest is the investigation of choice for the evaluation of any abnormality in a pulmonary fissure and adjacent areas of the lung.
