Summary:Primary pulmonary synovial sarcoma (PPSS) is an extremely rare intrathoracic soft tissue sarcoma, but there remains a notable absence of consensus guidelines for the diagnosis and treatment of these tumors. In this case, we reported a 35-year-old PPSS patient who received neoadjuvant chemotherapy consisting of adriamycin and ifosfamide combined with anlotinib, a multi-target tyrosine kinase inhibitor. Although initial response assessment was not encouraging, subsequent imaging studies revealed therapeutic efficacy. This case underscores the critical role of multidisciplinary team and neoadjuvant therapy in PPSS management, aiming to raise awareness of this rare malignancy.
