description [Objective] To investigate the effects of Remazolam and Propofol on hemodynamics and postoperative cognitive function during anesthesia induction in elderly patients undergoing laparoscopic surgery. [Methods] 116 elderly patients undergoing elective laparoscopic surgery in Yibin Hospital, West China Hospital, Sichuan University from December 2022 to March 2024 were selected as the study subjects and divided into observation group and control group according to random number table, with 58 cases in each group. Remazolam was used for anesthesia induction and maintenance in the observation group, and Propofol was used for anesthesia induction and maintenance in the control group. The hemodynamic indicators and the quality of recovery at two different time points were compared, and the occurrence of adverse reactions and cognitive function scores were recorded. [Results] In the observation group, the heart rate and mean arterial pressure immediately after tracheal intubation and 3 min after intubation were higher than those in the control group (P<0.05), the spontaneous respiratory recovery time, eye-opening time and extubation time were shorter than those in the control group (P<0.05), the cognitive function scores at 1 d and 3 d after operation were higher than those in the control group (P<0.05),the total incidence of adverse reactions was lower than that in the control group (P<0.05). [Conclusion] Compared with Propofol, Remazolam is used in elderly laparoscopic surgery, with more stable intraoperative blood flow fluctuations, faster postoperative recovery, and fewer adverse reactions. For patients with less cognitive function, it is suitable for elderly patients with relatively weak body and easily unstable hemodynamics.
