description This case report discusses a rare life-threatening fungal infection of the postoperative wound in deceased donor renal transplant recipient. A 43-year-old female patient, who is a known case of type 2 diabetes mellitus, hypertension, and chronic kidney disease underwent deceased donor renal transplantation and developed cutaneous mucormycosis at the postoperative wound site 12 days after renal transplantation. The patient had poor perioperative glycemic control. Successful management included timely identification of fungus, with early initiation of antifungal therapy (with local and systemic liposomal amphotericin B), extensive surgical debridement, and regular wound dressings. The case underscores the importance of strict glycemic control and strict aseptic precautions in the management of transplant recipients and vigilant follow-up posttransplant, particularly in diabetic patients. It highlights the need for a multidisciplinary proactive approach for timely diagnosis and effective management of mucormycosis and related complications.
