description We describe the case of a 45-year-old woman who developed a keyhole deformity of the anal canal following a cutting seton procedure to repair a transsphincteric anal fistula. As a result of using the Buried Island Transposition (BIT) de-epithelialized double-breasted fasciocutaneous flap technique, we were able to successfully repair the defect. Here, we present a systematic review of the surgical approaches and outcomes associated with the repair of anal keyhole deformities.
