Summary:Dissecting splenic artery pseudoaneurysm due to segmental arterial mediolysis (SAM) is a rare condition. We describe a case of direct percutaneous transgastric–transpancreatic thrombin injection into a dissecting splenic artery pseudoaneurysm due to SAM. The direct thrombin injection resulted in successful thrombosis of the pseudoaneurysm. At 1-month follow-up, the patient remained well with persistent thrombosis of the pseudoaneurysm.
