Risk factors and drug resistance analysis of fungal infections in maintenance hemodialysis patients

description 目的探讨维持性血液透析(maintenance hemodialysis,MHD)患者并发真菌感染的菌群分布特点、药敏特点以及发生真菌感染的危险因素,为积极预防,临床合理用药提供依据。方法选取我院肾脏内科2018年1月至2020年3月收治的495例MHD患者为研究对象,根据院内真菌感染的诊断标准,将发生真菌感染的71例患者纳入感染组,将未发生真菌感染的424例患者纳入非感染组。回顾性收集研究对象的临床资料,采用SPSS 26.0软件对我院MHD患者并发真菌感染的菌群分布特点、药敏特点以及危险因素进行统计分析。结果 495例MHD患者中,发生真菌感染者有71例,感染率为14.3%,真菌感染部位以肺部最常见,感染主要病原菌为白假丝酵母菌。药敏特点:真菌对两性霉素B、5-氟尿嘧啶、伏立康唑、氟康唑、伊曲康唑、棘白素类药物的耐药率分布为0、1.3%、15.5%、11.2%、14.1%、12.1%;真菌对两性霉素B、5-氟尿嘧啶的耐药率低于伏立康唑、氟康唑、伊曲康唑、棘白素类药物(P<0.05)。多因素Logistic回归分析结果显示糖尿病、白细胞计数<4×10<sup>9</sup>/L、抗生素使用时间≥7 d、使用激素和(或)免疫抑制剂是引起MHD患者发生真菌感染的独立危险因素(P<0.05,OR>1)。结论 MHD患者并发真菌感染部位以肺部最多见,感染菌群以白假丝酵母菌最多见,引起MHD患者真菌感染的危险因素较多,应针对危险因素采取措施积极预防,并根据药敏特点合理使用抗生素。
