Summary:Granulomatosis with polyangiitis, formerly known as Wegener’s granulomatosis, is a small- and medium-vessel autoimmune vasculitis that predominantly affects the respiratory tract and kidneys. As clinical presentation is often nonspecific, there is a heavy reliance on radiologic, laboratory, and biopsy findings for diagnosis. In this case, the patient initially presented with bilateral alveolar opacities and shock. Notably, the disease extended to the nasal mucosa and paranasal sinuses; however, the patient intriguingly reported no symptoms associated with the involvement of the upper respiratory tract.
