Summary:This case report documents a unique instance of Cladorrhinum bulbillosum, fungal keratitis in a pediatric patient responding remarkably well to antibiotic therapy. The presentation, management, and outcome of this molecularly diagnosed case highlight the unprecedented efficacy of moxifloxacin in the face of positive fungal culture. A 1-year-old child with a 1-month history of redness and watering presented with a corneal infiltrate clinically resembling a fungal infection. Microscopic examination revealed the presence of Gram-positive cocci, bacilli, and pus cells, complicating the diagnostic landscape. Despite the positive fungal culture, the patient exhibited a rapid and complete resolution of the infiltrate within a week following treatment with moxifloxacin eye drops. Molecular diagnostic tools confirmed the fungal isolate as Cladorrhinum bulbillosum, a rare cause of fungal keratitis known for its poor response to antifungal agents. This report, the first of its kind, underscores the potential role of antibiotic therapy in the management of specific cases of rare fungal keratitis, challenging conventional expectations and emphasizing the need for individualized treatment approaches.
