description Immune Recovery Uveitis (IRU) is paradoxical clinical worsening of intraocular inflammatory reactions, attributable to improved immune functions after starting Highly Active Anti Retroviral Therapy (HAART) in HIV patients. It is the ophthalmological manifestation of Immune Reconstitution Inflammatory Syndrome (IRIS) which involves various body systems. With the use of HAART, the incidence of Opportunistic infections has declined significantly. However, the development of Immune Recovery Uveitis in these patients can cause substantial vision loss, thereby posing a new challenge. This article reviews the etiopathogenesis, risk factors, clinical presentation and management of IRU.
