Effect of spironolactone on transmitral diastolic blood flow and course of chronic heart failure with preserved left ventricular ejection fraction

Existing recommendations on the mineralocorticoid/aldosterone receptor antagonists have been developed only for patients with systolic heart failure, and all outcome trials performed in patients with preserved ejection fraction failed to improve prognosis in this condition. Аim. To examine the ef...

description Existing recommendations on the mineralocorticoid/aldosterone receptor antagonists have been developed only for patients with systolic heart failure, and all outcome trials performed in patients with preserved ejection fraction failed to improve prognosis in this condition. Аim. To examine the effectiveness of spironolactone therapy in elderly patients with chronic heart failure NYHA I–II with preserved left ventricular ejection fraction. Methods and results. Peculiarities of transmitral diastolic blood flow was studied with Doppler echocardiography. 6-min walk test and NT-proBNP was performed for the assessment of clinical course improvement in such patients. Conclusion. It was found that spironolactone therapy improves mitral diastolic blood flow, reduces concentration of NT-proBNP, which correlates with improved clinical course of heart failure.
