description Determination of erythrocyte cholinesterase (AChE) and plasma cholinesterase (BChE) activities in human blood has been suggested for the diagnosis of organophosphate pesticide (OP) exposure and intoxication. The original colorimetric Ellman procedure is widely used for this purpose with a variety of modifications as per the requirements. We made certain modifications to the Ellman procedure to make the assay simple and rapid, which included pH of the assay medium (7.2), and use of kinetic mode for optical measurements to avoid the addition of inhibitors for stopping the enzyme reaction of the assay. Common substrate (acetylthiocholine) as well as reagents were used both for the AChE and BChE activity. Precision, linearity of the colorimetric measure and stability (up to 56 days) of the modified color reagent used in the proposed assay were checked. Linearity with high correlation was observed and the color reagent was found to be quite stable even after 56 days when stored in the dark at 40℃. On comparison with the Ellman method, high correlation was observed suggesting that the proposed method is capable of giving results within the bounds of clinical requirements. Organophosphate pesticide poisoning in developing countries has high morbidity and mortality rates, as facilities for diagnosis or immediate treatment are not easily available. This modified approach provides a simple and rapid way for the determination of AChE and BChE from blood, which can be useful for diagnosis and management of OP-poisoned patients or in health surveillance of workers at risk of OP pesticide exposure particularly in Asian countries.
