Summary:Complete atrioventricular block (AVB) has not been previously reported in patients with Alport syndrome with normal kidney function. We describe a rare case of a young man with Alport syndrome who developed complete AVB. Cardiac magnetic resonance imaging revealed no signs of inflammation or other abnormalities. Whole-exome sequencing identified several variants, but none alone could explain the heart block. We hypothesize that AVB may be a potential complication of Alport syndrome or the result of multiple variants. A permanent pacemaker was implanted, and a 6-month follow-up confirmed the irreversibility of the AVB.
