| description | Objective To investigate the effects of different doses of the first dose of heparin anticoagulation effect in maintaining hemodialysis patients.Methods 62 cases of hemodialysis patients were randomly divided into experimental group(A group = 30) and control group(group B=32).At the beginning of each dialysis A group from the venous side to give the first dose of heparin 28~32 mg(about 0.5 mg/kg),the maintenance period of heparin added 6~10 mg per hour by heparin pump;At the beginning of each dialysis B group from the venous side to give the first dose of heparin 14~20 mg(about 0.3 mg/kg),the maintenance period of heparin added 8 ~12 mg per hour by heparin pump,Simultaneous monitoring of two groups of 0 h,0.5 h,2 h,3.5 h and 4 h activated coagulation time(ACT) and the change curve,observation of treatment process piping and dialyzer coagulation,the average time of puncture point compression hemostasis,tissue and organ bleeding within 24 h after hemorrhage and online kt/V situation.Results There was a significant difference between the two groups of patients with the first dose of heparin and maintenance period,but there was no difference in the total amount of heparin.Two groups were able to successfully complete hemodialysis,the dialysis adequacy of the experimental group was better.Compared with the control group,0.5 h and ACT 2 h in the experimental group were significantly longer than those in the control group,and 3.5 h and 4 h ACT in the dialysis was significantly shorter,two groups had significant difference(P<0.01);Through self comparison of 0.5 h and 3.5 h ACT two,the experimental group decreased significantly(P<0.05),the control group had no significant difference;Two groups of patients with dialyzer coagulation,Puncture point compression hemostasis,tissue and organ bleeding within 24 h after hemorrhage the experimental group was better than the control group(P<0.01).Conclusions The key to the smooth progress of hemodialysis is to select the appropriate dose of the first dose of heparin and heparin of the maintenance period,group with a large amount of the first dose of heparin anticoagulation effect is good,the dialysis adequacy is good,and less bleeding after dialysis,. |
