Summary:Presurgical infant orthopedics (PSIOs) play a vital role in addressing orthopedic conditions in infants, particularly cleft lip and palate (CLP), which significantly impact orofacial functions. This article explores the etiopathogenesis of CLP, highlighting genetic and environmental factors contributing to its occurrence. It emphasizes the interdisciplinary nature of PSIO, involving pediatric orthopedic surgeons, plastic surgeons, speech therapists, dentists, and audiologists in devising tailored treatment plans. The article delves into various PSIO techniques, including lip adhesion, taping, passive feeding plates, and active appliances such as nasoalveolar molding (NAM) and its modifications. NAM, pioneered by Grayson and Cutting, stands out for its ability to correct nasal and alveolar deformities simultaneously, reducing surgical challenges and improving outcomes. Modifications such as the Figueroa method and Bennun’s dynamic presurgical nasoalveolar remodeling offer innovative approaches to enhance results. Advancements in PSIO, such as three-dimensional printed appliances and nostril retainers, showcase the evolution of treatment modalities. The article also traces the historical development of PSIO, highlighting key milestones from ancient surgical closures to modern techniques such as the Latham appliance and NAM therapy. Overall, PSIO epitomizes a holistic approach to CLP management, emphasizing early intervention, interdisciplinary collaboration, and customized treatment strategies for optimal functional and esthetic results.
