| Summary: | This study aimed to elaborate an instrument for nursing consultations for hypertensive patients attended in the Family Health Strategy, based on Horta’s model. The instrument covers identification data, medical history, physical examination, diagnosis, expected results, prescription and nursing evolution. An instrument to guide nursing consultations with hypertensive patients allows the identification of individual and social variables that influence the evolution of hypertension, contributing to lifestyle changes and favoring a care plan. In the context of nursing practices in the family health strategy, nursing consultations facilitate nurse-hypertensive patient relations. The importance of data records is highlighted, focusing on high-quality data production and the enhancement of external actions, educative actions and group and collective activities. The use of the International Classification for Nursing Practice is suggested to standardize the language nurses use in the Family Health Strategy, as well as research development with a view to the applicability of this model. |
