| description | This is a case report of a 71-year-old lady with long-standing rheumatoid arthritis who presented with sudden onset of gross hematuria and urinary retention. She was found to be hemodynamically unstable and necessitating blood transfusions. Ultrasonography revealed normal-sized kidneys with evidence of cystitis. Further workup was pursued including cystoscopy which revealed diffuse petechial hemorrhages in the bladder. Histopathology showed evidence of extracellular eosinophilic material within the wall of vessels and interstitium and Congo red staining confirmed amyloidosis. Fulguration of the bladder mucosa was performed with resolution of hematuria. The association between rheumatoid arthritis and secondary amyloidosis is known, but bladder amyloidosis is a rare manifestation. Our case report describes this rare occurrence along with a review of the existing literature on this subject and provides insight into the presentation and management of this life-threatening condition. |
