description Purpose: To investigate the reliability of the duochrome test among different refractive errors under different lighting condition. Materials and Methods: A prospective, comparative study was conducted among 40 subjects comprising of 19 myopic and 21 hyperopic patients. Each participants underwent duochrome test under three lighting conditions. Photopic, Mesopic and Scotopic lighting were defined as the conditions in which the illuminance level was measured with two light sources, one light source and a dark room with LED display respectively. Each participant underwent examinations in two phases. Phase – I ocular examinations included visual acuity assessment, objective refraction and subjective refraction. Maximum plus and minimum minus rule was used for the end point of monocular subjective refraction followed by duochrome testing under recommended conditions (i.e. scotopic condition). Phase – II included the pupil size measurement and duochrome test under two lighting conditions (photopic and mesopic). Bland–Altmann plot was used to evaluate the agreement between the measurements of spherical equivalent refraction under different conditions. Results: The mean pupil size of myopic participants was greater than hyperopic groups under three lighting conditions. The Bland–Altman plots for the agreements in duochrome refraction between scotopic and other two lighting conditions (scotopic and mesopic) for the myopic group showed a good level of agreement based on the 95% limits of agreement, whereas a low level of agreement observed for the hyperopic group. Conclusion: Duochrome test is reliable during myopic refraction irrespective of the lighting condition of the examination room.
