description Electroconvulsive therapy (ECT) is a well-established intervention for severe psychiatric disorders, particularly in treatment-resistant cases. This study investigates the therapeutic outcomes of severe psychiatric pathologies treated with varying ECT modalities, focusing on the evolution of symptomatology in relation to specific ECT parameters and patient-specific clinical features. A detailed analysis was conducted on ten cases involving severe neuropsychiatric conditions treated with ECT. The methodology incorporated a comprehensive review of psychiatric histories, comorbidities, and previous pharmacological and therapeutic interventions. Particular emphasis was placed on assessing the relationship between variations in ECT parameters—such as electrode placement, seizure threshold, and stimulation intensity—and clinical outcomes, including behavioral and symptomatic changes post-ECT. The findings demonstrate the heterogeneity of responses to ECT across the 10 cases, highlighting the influence of tailored ECT parameters on therapeutic efficacy. Cases illustrate the dynamic interplay between individual patient profiles and treatment modalities, with specific improvements noted in psychotic symptoms, affective dysregulation, and suicidal ideation. Severe psychiatric disorders exhibit diverse responses to ECT, with outcomes closely linked to individual patient profiles and specific treatment parameters. Customizing ECT protocols proved essential for achieving optimal symptom improvement, particularly in cases with significant comorbidities.
