Summary:Purpose: The purpose of the study was to evaluate the efficacy of electrocautery and conjunctivoplasty with fornix suturing in managing conjunctivochalasis (CCH). Materials and Methods: A prospective interventional study was conducted including patients with symptomatic CCH who were not relieved by medications. Patients with CCH were divided into 2 groups. In group A, CCH reduction was done using radiofrequency electrocautery in fulguration mode. In group B, CCH reduction was done using conjunctivoplasty with fornix conjunctival suturing. Outcome measures included Schirmer’s test, tear film breakup time (TBUT), tear film height (TFH), tear meniscus area (TMA), and CCH area at day 1, day 30, and day 90. Results: One hundred eyes were included in the study (50 eyes in each group). The ST values did not show a significant difference between the groups at different postoperative periods (P = 0.42, right eye [RE]; P = 0.38, left eye [LE]). TBUT, tear meniscus height, TMA, and CCH area was significantly improved in both groups at postoperative day 90 (P < 0.001). There was no significant difference in the TBUT values for LE (P = 0.38) and RE (P = 0.54) between the groups. The electrocautery group showed increased TFH for both RE (P = 0.02) and LE (P = 0.01) compared to the fornix suture group. There was no significant difference in TMA between both groups for RE (P = 0.44) and LE (P = 0.37). Furthermore, there was no significant difference in CCH area between the groups for RE (P = 0.94) and LE (P = 0.89). Conclusion: Both procedures have resulted in improvement in the patient’s tear film parameters and can be chosen according to the surgeon’s preference.
