Summary:Introduction: Homeless individuals face multifaceted challenges in reintegration, yet physical activity remains largely absent in rehabilitation models within Southeast Asian urban settings like Medan and Penang. Objective: This study explores the role of physical activity in supporting the reintegration of homeless individuals through comparative insights from shelters in Medan and Penang. Methodology: A qualitative, comparative case study approach was used involving 26 participants through interviews, FGDs, and document analysis. Thematic analysis identified patterns in perceptions, institutional responses, and rehabilitation practices in both cities. Discussion: Findings show that physical activity is overlooked despite its potential to enhance psychosocial well-being, rebuild identity, and structure routines. Existing models are outdated, fragmented, and miss long-term reintegration goals. Conclusion: Integrating physical activity into rehabilitation offers a low-cost, transformative approach for social reintegration. A shift toward inclusive, holistic policies is needed.
