description Ventricular free wall rupture is a rare complication of acute myocardial infarction (MI). It can present in acute lethal forms like a blowout type or subacute form where the blood clot, in rare circumstances, seals the defect. A 72-year-old male presented with MI and had pericardial effusion with a clot on the echocardiogram. He was diagnosed with a left ventricular free wall rupture (LVFWR) and proceeded with emergency surgery, where the rupture site was closed with pericardium. The surgeon retrieved a large clot in the pericardium that was sealing the perforation site – preventing cardiac tamponade. Hence, it is necessary to identify sealed free wall rupture cases.
