We describe a case of a 65 year old female who presented to us with a mass lesion on the left upper eyelid. Incisional biopsy from the tumor mass confirmed the diagnosis of basal cell carcinoma (BCC). The primary tumor was excised and lid reconstructive surgery was performed. She also had a nodulo-ulcerative lesion over the dorsum of her left hand. Biopsy from the hand lesion showed BCC features consistent with the primary tumor, confirming the diagnosis of MBCC. This case highlights the fact that although BCC rarely metastasizes, ophthalmologists should be aware of its metastatic potential, especially in the presence of high risk clinical and histopathologic factors.
