Summary:[Objective] To analyze the clinical efficacy of the Transanal Opening of Inter-sphincteric Space (TROPIS) procedure for treating high inter-sphincteric fistula-in-ano. [Methods] A retrospective analysis was conducted on the clinical data of 28 patients with high inter-sphincteric fistula-in-ano who underwent TROPIS at the First Affiliated Hospital of Guangzhou University of Chinese Medicine from September 2018 to July 2020. The anal function of patients before and after surgery was compared, postoperative complications were observed and recorded, and the clinical efficacy of the procedure was evaluated. [Results] A total of 28 patients were included in this study, all of whom were cured (100%). There were no recurrences during the 6-month follow-up period. There was no statistically significant difference in the Wexner incontinence score before surgery and 3 months after surgery (P>0.05). Two patients experienced wound bleeding on the first and third days postoperatively, which was controlled with tamponade using T-shaped gauze, with no significant bleeding afterward. No other complications were observed in the remaining patients. [Conclusion] TROPIS for the treatment of high inter-sphincteric fistula-in-ano has good clinical efficacy and minimal impact on anal function.
