Infection-related Glomerulonephritis with Reactive Arthritis in a Patient with COVID-19 Infection
Renal involvement in coronavirus disease-19 (COVID-19) varies from the most common acute tubular injury to the rare immune-mediated glomerular lesions. | description | Renal involvement in coronavirus disease-19 (COVID-19) varies from the most common acute tubular injury to the rare immune-mediated glomerular lesions. We present a 44-year-old female who presented with acute nephritic syndrome along with endocapillary proliferative glomerulonephritis; co-dominant C3 deposits in renal histology and hypocomplementemia. She tested positive for COVID-19 polymerase chain reaction assay in the throat swab and had mild lung involvement. Three weeks later, she developed reactive arthritis. Her symptoms resolved with analgesics and physiotherapy. On follow-up, her renal parameters and complements got normalized. She tested positive for anti-COVID-19 spike protein antibodies. To the best of our knowledge, this is the first case report of infection-related glomerulonephritis in a COVID-19-infected patient from India, along with reactive arthritis. It is likely that severe acute respiratory syndrome-coronavirus 2 causes heightened systemic immune activation, thereby mediating the pathogenesis of infection-related glomerulonephritis |
