description The coexistence of aortic occlusion and a horseshoe kidney is an extremely rare phenomenon. This paper presents the case of a 64-year-old female patient who was admitted to the vascular surgery department due to critical lower limb ischemia. Based on the clinical presentation and imaging studies, which revealed abdominal aortic occlusion and a horseshoe kidney, the patient was qualified for an aortobifemoral bypass grafting via a midline abdominal incision. The patient was discharged home in good general and local condition. Due to the anatomical complexity of the horseshoe kidney, the described surgery poses a challenge even for experienced surgeons.
