Summary:Tilted optic disc and tilted retina are rare anatomical variations that may lead to visual disturbances and diagnostic challenges. We outline the case of a young 22-year-old male patient, who had diminished vision in his left eye. His right eye vision was 6/6, while his left eye vision was 6/60, best corrected to 6/9. Intraocular pressure was 16.0 mmHg in the right eye and 14.0 mmHg in the left eye. The anterior segment was within normal limits. Fundus examination revealed a tilted optic disc in the left eye, and optical coherence tomography imaging showed a tilted retina. Visual field analysis (30-2) revealed mildly decreased sensitivity in the inferior quadrant in the left eye. Finally, the diagnosis of left eye compound myopic astigmatism with tilted disc and retina was made. The case highlights the need for thorough evaluation in such presentations to rule out associated conditions such as myopic degeneration and secondary glaucomatous damage.
