Summary:Non-Hodgkin lymphoma (NHL) is the fourth most common malignancy among men and women with its incidence increasing every year at the rate of 3%-4%. Involvement of Cavernous sinus in NHL is very rare. We report a case of Retro-peritoneal NHL treated successfully with chemotherapy reporting after 6 months with ptosis, proptosis in left eye, headache and diplopia. MRI brain showed only thickened Optic nerve and was opined as optic neuritis left eye. Due to strong clinical ophthalmic picture suggesting of orbital apex or cavernous sinus involvement a Computerised Tomography (CT) head was done which showed involvement of left cavernous sinus. She was treated with a course of methylprednisolone and chemotherapy which lead to complete resolution of her symptoms dramatically. Orbital lesions may be the first clinical manifestations and a proper ophthalmic examination may give the first indication of cavernous sinus involvement
