description Abstract The sociodemographic conditions and natural characteristics of the state of Chihuahua, in Mexico, have historically posed challenges for the implementation and monitoring of malaria control programs. The absence of systematized documentation on malaria transmission has hindered technical planning for elimination. The objective was to identify and characterize transmission patterns in malaria-endemic localities in the state and analyze sociodemographic variables associated with transmission in these communities over the last twenty years (2000–2022). This study presents a retrospective analysis using surveillance data from malaria databases and entomological control activities by locality in the state of Chihuahua, provided by the Epidemiology Subdirectorate of the Chihuahua Health Services, along with other open-access databases. The analysis was conducted to determine the prevalence of malaria transmission in all foci. Additionally, semi-structured interviews were conducted with operational staff of the malaria control program. Based on observations of the variation in the epidemiological trend of active outbreaks from 2000 to 2022, it was determined that the variation in cases per year was associated with population changes in the localities, predominantly due to limitations on transit associated with local economic activities. Social, environmental, and clinical factors such as migration, geographical isolation and ethnic beliefs and traditions associated to changes in malaria transmission were identified. This is the first study to integrate the operational experience of the control and surveillance program with a comprehensive approach to the social context in the region, which could support the elimination program.
