Incidence and implications of new-onset fever in pediatric intensive care unit patients: A single center prospective observation study from North India

Summary:Background: New-onset fever (NOF) in pediatric intensive care unit (PICU) is a common occurrence and has an impact on patient outcomes. The PICU patients are especially at risk of developing NOF due to various interventions and devices such as endotracheal tube, central line, and urinary catheters. This study aims to estimate the incidence of NOF among the children admitted to PICU and to determine its effect on length of hospital stay. Subjects and Methods: This was a hospital-based prospective observational study done over 1 year, children aged 1 month–18 years admitted to PICU and developing NOF during the study period were enrolled in the study. Results: The incidence of NOF was 6.83% and catheter-associated urinary tract infection was the most common cause for NOF in 18.3% patients, followed by central line-associated bloodstream infection and ventilator-associated pneumonia causing NOF in 14.2 and 12.5% patients, respectively. The median duration of PICU stay was significantly more in patients with NOF (15 [10–22] vs. 7 [4–10] days [P < 0.001]) than those without NOF. Conclusions: NOF in PICU patients was predominantly due to infections, with device-related infections being the most common cause. Patients with NOF had longer duration of PICU stay as compared to patients without NOF.
