description In this case study, a bilateral acute angle closure due to choroidal effusion was the initial manifestation of a previously undiagnosed hantavirus infection (hemorrhagic fever with renal syndrome). The diagnostic and treatment algorithms are described. Advanced diagnostic technologies and a systematic approach to the analysis of atypical angle closure allowed not only to identify the cause of clinical presentations — choroidal effusion and anterior displacement of the lens–iris complex, but also to reveal the underlying systemic disease. The clinicians’ awareness of acute angle closure mechanisms is important in preserving the patient’s vision and timely diagnosing potentially life-threatening conditions.
