| Summary: | ObjectiveTo study the practice and effectiveness of the "Integrated Control and Prevention of Three Highs (hypertension, hyperglycemia, and hyperlipidemia) and Six Diseases (coronary heart disease, stroke, renal lesions, retinal lesions, peripheral neuropathy, and peripheral vascular lesions)" chronic disease management strategy in Chengyang district, Qingdao city, and to make policy recommendations for improving chronic disease prevention and control in China. MethodsData on management and referral of the "Three Highs" patients in 2019 and 2021 ( before and after the implementation of the "Integrated Control and Prevention of Three Highs and Six Diseases" management strategy) were provided by Chengyang District Health Commission of Qingdao city. Key informant interviews were conducted to collect information on the operation mode, two-way referral mechanisms, and information platform development in the implementation of the strategy. Qualitative and quantitative analyses were conducted in the study. Results Among the "Three Highs" patients in the district, the standardized treatment rate and control rate were increased but the incidence of complications was effectively controlled after the implementation of the "Integrated Control and Prevention of Three Highs and Six Diseases" management strategy. ConclusionsThe "Integrated Control and Prevention of Three Highs and Six Diseases" strategy is of certain promotion and reference value on account of the provision of health centered continuity management services, integration and synergy of primary health care services, and improvement of service efficiency and capacity of grassroots medical and health institutions under the implementation of the strategy. |
