| description | Background. Forensic medical assessment of the quality of medical care is a critically important task for improving the healthcare sector. Surgical practice is associated with high risks to the lives of patients, as it combines the stage of planning surgical intervention, its direct implementation with the involvement of an anesthesiologist, the postoperative period and subsequent recovery. Diseases that primarily require surgical intervention and the lack of treatment of which can lead to death are pathologies of the gastrointestinal tract. At the same time, the health of the nation is determined by the level of health of children and adolescents, because this is the basis of economic and social life and development of the state as a whole. Analysis of fatal cases of deaths from diseases of the gastrointestinal tract is an indirect marker of the quality of surgical medical care for children.
Purpose – аnalysis of the dynamics of cases of non-violent child mortality from diseases of the gastrointestinal tract according to the data of forensic medical reporting, which is formed by the DSU «Main Bureau of Forensic Medical Examination of the Ministry of Health of Ukraine» (DSU GB) for 7 years as an indirect marker of the quality of surgical medical care for children.
Materials and Methods. The object of the study was the cases of forensic medical examinations of non-violent childhood mortality from gastrointestinal diseases, the total number of non-violent mortality and overall mortality from gastrointestinal diseases during 2016–2022 in Ukraine. Infectious, urological, pulmonary, other diseases, oncopathology, tuberculosis, which caused non-violent death, were excluded. To assess quantitative indicators, the percentage expression (P) was used, which was calculated as the proportion of variants with the studied features (n) to their total number (N). In addition, the representativeness error (mP) was determined with a confidence interval (CI) at the level of 95%.
Results. Analysis of fatal cases of gastrointestinal diseases in children, which were the object of the study of forensic experts, indicates their significant fluctuation from 0.00 to 9.3% depending on age and year in relation to mortality from this pathology, which on average is 0.27 ± 0.02% of the total level of non-violent death. The study of this indicator taking into account the age of children revealed the most vulnerable age groups of 12–18 years and 1–6 years, which require special attention from surgeons. In contrast, the age group of 7–11 years in half of the studied period demonstrated zero mortality rates, which indicates a sufficient level of medical care.
Conclusions. The findings among those who died from gastrointestinal diseases, which were the subject of a study by forensic experts, of the most vulnerable age groups of 12–18 years and 1–6 years indicate the need for special attention to the treatment of severe surgical cases, as well as the importance of timely detection of pathologies to prevent fatal outcomes. The low mortality rate in the age group of 7–11 years, which indicates a high quality of medical care, requires an interdepartmental analysis to identify the factors that contributed to this.
