description Objective To observe the effect of losartan on urinary protein spectrum in patients with idiopathic membranous nephropathy(IMN) to clarify partial therapeutic mechanism.Methods The 79 IMN patients were randomly divided into the observation group(n = 42 patients) and the control group(n= 37).After conventional treatment for one month,the patients in observation group were given losartan for three months(100 mg daily),and those in control group conventional therapy continuously.The urinary β2-microglobulin to creatinine ratio(β2-MG/Cr),the urinary retinol binding protein to creatinine ratio(RBP/Cr),the urinary protein to creatinine ratio(PRO/Cr),the urinary podocyte apical membrane protein to creatinine ratio(PCX/Cr),the urinary albumin to creatinine ratio(Alb/Cr),and immunoglobulin G to creatinine ratio(IgG/Cr) before and three months after treatment were detected.Results After treatment with losartan,PCX/Cr,Alb/Cr and PRO/Cr were obviously decreased(P<0.01),but there was no significant difference in β2-MG/Cr,RBP/Cr and IgG/Cr(P>0.05) in the observation group.There was no significant difference in all ratios before and after treatment in the control group(P<0.05),except PRO/Cr and Alb/Cr(P<0.05).As compared with the control group,PCX/Cr,Alb/Cr and PRO/Cr ratios were decreased(P<0.05),and there was no significant difference in β2-MG/Cr,RBP/Cr and IgG/Cr ratios(P>0.05) after treatment for three months in the observation group.Conclusions Losartan had a protective effect on charge barrier in patients with idiopathic membranous nephropathy,thus effectively reducing urinary protein excretion,but had no effects on glomerular basement membrane structural damage and renal tubular injury.
