Summary:ObjectiveTo explore the correlation of gastrointestinal symptoms with sleep disorder, anxiety and depression in hemodialysis patients based on the Least Absolute Shrinkage and Selection Operator (LASSO) -Logistic regression.MethodsA total of 101 hemodialysis patients admitted to the First Affiliated Hospital of Anhui Medical University(Anhui Public Health Clinical Center)from July 2022 to July 2024 were selected. Gastrointestinal Symptom Rating Scale (GSRS) was used to evaluate the hemodialysis patients, and they were divided into symptom group (60 cases) and asymptomatic group (41 cases) according to the gastrointestinal symptoms. Pittsburgh Sleep Quality Index (PSQI), Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) were used to evaluate the patients' sleep disorders (8 cases in normal group, 10 cases in mild group, 30 cases in moderate group and 12 cases in severe group), anxiety (14 cases in normal group, 24 cases in mild group, 13 cases in moderate group and 9 cases in severe group) and depression (11 cases in normal group, 23 cases in mild group, 18 cases in moderate group and 8 cases in severe group), respectively. General data of two groups of patients were compared. LASSO-logistic regression was used to analyze the influencing factors for gastrointestinal symptoms in hemodialysis patients. Spearman correlation was used to analyze the correlation of GSRS score with sleep disorders, anxiety and depression in hemodialysis patients.ResultsHemoglobin (HB) [(107.22±15.04)g/L <italic>vs</italic> (117.12±15.42)g/L], serum creatinine (Scr) [(829.36±152.47)μmol/L <italic>vs </italic>(932.15±163.78)μmol/L], and blood urea nitrogen (BUN) [(21.35±3.79)mmol/L <italic>vs </italic>(24.15±3.86)mmol/L] in the symptomatic group were significantly lower than the asymptomatic group (<italic>P</italic>&lt;0.05), while the PSQI[12.5(10.0,14.0)points <italic>vs</italic> 9.0(5.0,13.5)points], SAS score [(57.00±8.81)points <italic>vs </italic>(48.02±6.46)points], and SDS score [(60.93±7.54)points <italic>vs </italic>(48.98±7.24)points] were significantly higher than the asymptomatic group (<italic>P</italic>&lt;0.05). LASSO-Logistic regression analysis showed that low Hb (<italic>OR</italic>=0.505, 95%<italic>CI</italic>: 0.294-0.922), Scr (<italic>OR</italic>=0.515, 95%<italic>CI</italic>: 0.284-0.904), BUN (<italic>OR</italic>=0.496, 95%<italic>CI</italic>: 0.245-0.880), high PSQI (<italic>OR</italic>=2.061, 95%<italic>CI</italic>: 1.268-4.650), SAS score (<italic>OR</italic>=2.002, 95%<italic>CI</italic>: 1.112-4.107) and SDS score (<italic>OR</italic>=2.042, 95%<italic>CI</italic>: 1.086-3.967) were the influencing factors for gastrointestinal symptoms in hemodialysis patients (<italic>P</italic>&lt;0.05). GSRS score was significant different between asymptomatic group versus mild, moderate and severe groups [3.00(2.00,3.75)points <italic>vs</italic> 4.00(3.00,6.50)points、8.00(4.75,11.25)points、10.00(6.00,14.00)points], anxiety group versus mild, moderate and severe groups[2.0(3.0,4.0)points <italic>vs</italic> 5.0(4.0,8.0)points、9.0(6.0,14.0)points、12.0(7.0,14.5)points], and depression group versus mild, moderate and severe groups [4.00(2.00,5.00)points <italic>vs</italic> 5.00(4.00,8.00)points、8.00(6.00,14.25)points、10.50(8.25,14.75)points] (<italic>P</italic>&lt;0.001). Spearman correlation analysis showed that GSRS score of hemodialysis patients was positively correlated with sleep disorder, depression and anxiety (<italic>P</italic>&lt;0.001).ConclusionGSRS score of hemodialysis patients is related to low levels of serum Hb, Scr, BUN, as well as high scores of PSQI, SAS, and SDS. The gastrointestinal symptom scores of patients are significantly correlated with the severity of sleep disorders, anxiety, and depression.
