description <strong>Foundation:</strong> the nurse is currently, the professional with the best profile, to be able to carry out health promotion and prevention in patients diagnosed with chronic diseases during home visits. <br /><strong>Objective</strong>: to describe the work of nursing in home visits to patients with chronic diseases at the Sigsipamba Health Center. <br /><strong>Methods:</strong> a descriptive and cross-sectional study was carried out with patients from the Sigsipamba Health Center, in the San Francisco de Sigsipamba Parish, Pimampiro Canton, Imbambura Province, Ecuador, from April to November 2023. The sample consisted of 77 patients. Demographic and clinical variables were studied and descriptive statistics were used. <br /><strong>Results:</strong> the mean age of the population was 54.4 years. The female sex predominated with 39 patients (50.6 %). Arterial hypertension was the most representative chronic disease with 46 patients (59.7 %), followed by smoking with 44 patients (57.1 %). The most reported purpose of the visit was disease monitoring and follow-up by 33 patients (42.8 %). Once a month was the predominant frequency of visits with 69 patients (89.6 %). The main activity during visits was the control of vital signs in 60 patients (77.9 %). <br /><strong>Conclusions:</strong> the nursing work during home visits should include disease monitoring and follow-up, health education among its objectives.
