This study may supply scientific evidences to guide clinicians in deciding whether chlorhexidine bathing is a suitable preoperative care in primary TKA. Materials and Methods: A total of 2,305 primary TKA cases were enrolled, and two groups were divided: those who employed chlorhexidine gluconate (CHG) bathing on the night before the surgery (CHG group) and those who did not (control). In this cohort, there were 576 cases in the chlorhexidine group and 1,729 cases in the control group. Results: The results showed that the incidence of PJI was not significantly different between the two groups, with an overall infection rate of 0.52%. In this cohort, PJI occurred in 12 cases of primary TKA, with an overall infection rate of 0.52%. Eight knees in the control group (0.46%) and four knees in the CHG group (0.69%) developed PJI. Nevertheless, the PJI incidence analyzed by chi-square test did not differ significantly between the groups. Conclusions: The study suggests that using chlorhexidine bathing on the night before TKA does not significantly reduce the risk of PJI in primary TKA. However, larger prospective studies are needed to confirm this conclusion.
