Soft cataracts are difficult to chop. Phacoemulsification within the capsular bag is prone to complications. Hence, we are proposing a novel technique anterior chamber dialer divide (ACDD) for safer, efficient nuclear management of soft cataract.
Design:
Prospective randomized comparative study.
Materials and Methods:
One hundred patients were randomly assigned to Group A novel ACDD phacoemulsification and Group B conventional phacoemulsification. Operated by a single surgeon. Intraoperative parameters including ultrasound time (UST), effective phacoemulsification time (EPT), balanced salt solution volume utilized, and surgeon’s comfort were evaluated. Three months follow-up was done and best-corrected visual acuity, central corneal thickness (CCT), and endothelial cell count were assessed.
Results:
ACDD group utilized less UST and EPT. The mean endothelial cell loss in Group A was 13.86% and in Group B was 17.72% and the difference with Group B was statistically significant. The mean CCT on 3rd month postoperative day in Group A patients was 509.14 and in Group B patients was 530.27, and the difference was statistically significant (P < 0.001).
Conclusion:
ACDD allows easy and fast emulsification of soft cataracts. This technique utilizes less phaco time and energy thereby less damage to corneal endothelium. It has better surgeon comfort. All these factors result in a better visual outcome. |
