Two similar Class III anterior crossbite cases illustrate how the skeletal anchorage can prevent protrusion after anterior crossbite correction and the need for four- premolar extractions, as in the first case. The use of extra-radicular miniscrews at the infrazygomatic crest and buccal shelf are recommended for whole arch distalization of the maxillary and mandibular arches to reduce protrusion after anterior crossbite correction. It is an effective and efficient treatment alternative to extraction therapy for the protrusion after anterior crossbite correction. Long-term follow-up records show encouraging results supporting this paradigm shift in anterior crossbite correction with extra-radicular miniscrews.
