description Background and purpose: Considering the negative effects of menopause on oral health and the importance of quality of life in women, this study was conducted with the aim of investigating oral health-related quality of life (OHRQoL) in menopausal women. Materials and methods: This descriptive, analytical, cross-sectional study was conducted during the academic year 2023–2024 on 193 menopausal women in Sari, Iran. The DMFT (Decayed, Missing, and Filled Teeth), PI (Plaque Index), GI (Gingival Index), and SBI (Sulcus Bleeding Index after probing) were recorded. The data were entered into SPSS version 23 and analyzed using the Mann–Whitney U test, Kruskal–Walli’s test, and Spearman’s correlation coefficient. Results: The average OHRQoL score was 11.6 ± 9.77 using the ADD method and 3.51 ± 3.12 using the SC method. The average OHRQoL score was significantly higher in individuals with prostheses (P = 0.037) and in those reporting dry mouth (P = 0.016) compared to others. Additionally, a significantly higher quality of life score was observed in participants with higher PI index values (P = 0.036). The quality-of-life score showed a direct and significant correlation with the DMFT index (P = 0.012, r = 1.180) and dry mouth score (P< 0.001, r= 0.260), whereas an inverse but non-significant correlation was found with age (P= 0.639, r= -0.034). Conclusion: Based on the results, the oral health-related quality of life in postmenopausal women was generally good. However, denture use, presence of dry mouth, and higher average PI and DMFT scores had a negative impact on OHRQoL, whereas age, GI, and SBI indices did not have a significant effect on OHRQoL.
