Summary:Abstract Background Lumbar plexus schwannomas are rare tumors which could prove to be elusive in patients who are pregnant. They may present with back, pelvic, flank or even radicular pain which may masquerade as common ailments experienced during pregnancy. Case presentation We report a rare case of a lumbar plexus schwannoma initially presenting during early pregnancy with back pain and lower limb radiculopathy in a 23 year-old patient. Subsequent imaging demonstrated that her symptoms were not result of the physiological changes of pregnancy but a consequence of spinal tumors and a lumbar plexus nerve sheath tumor. She eventually underwent surgeries to address her spinal and lumbar plexus tumor which was followed by resolution of her symptoms. Conclusion Lumbar plexus and retroperitoneal schwannomas are ideally treated by complete resection especially if they are of considerable size and symptomatic. However, due to its rarity, a general consensus for treatment of these lesions is not available. In patients who are pregnant, a high index of suspicion should be maintained when symptoms appear incongruent or disproportionate.
