Summary:Background: Simultaneous bilateral femoral neck fractures are rare and often underdiagnosed, typically affecting patients with significant comorbidities. Although these fractures are rarely reported in the literature, they can occur in cases of severe skeletal trauma, such as motor vehicle accidents or electrical injuries. Prompt recognition and early intervention are essential, as delayed diagnosis can significantly increase morbidity and mortality.Case Presentation: We report the case of a 79-year-old Caucasian male with multiple medical comorbidities, who developed bronchopneumonia complicated by sepsis. Following treatment and discharge, the patient returned one week later with bilateral hip pain. Physical examination revealed functional disability and external rotation of both lower limbs, and radiographs of the hips and pelvis identified bilateral femoral neck fractures. The fractures were successfully managed with hip arthroplasty.Conclusion: This case underscores the importance of vigilance for such injuries in patients with complex medical conditions and highlights the need for timely diagnosis and intervention to prevent further complications.
