description Abstract Background Acute gastric perforations beyond the neonatal age are very rare and are associated with a significant morbidity and a high mortality rate. The presence of a left sided eventration of diaphragm may act as a contributory factor leading to volvulus and subsequent ischemic necrosis. Case summary We present an 11-month-old child with an isolated acute gastric perforation and eventration of left hemi-diaphragm with no other significant medical history. Conclusion Acute gastric perforation in infants with eventration of left diaphragm carries a high mortality rate and should be suspected in infants with recurrent non bilious vomiting and abdominal distention.
